Consent Form - LOVE CARE GLOBAL HOME NURSING

Must be signed before care begins.

Patient Consent

I consent to receive home nursing care from a verified nurse assigned via LOVE CARE GLOBAL. I understand hourly pricing, Start/End time, quantity, and total hours calculation. I provide accurate health information, address, GPS, phone with country code.

Responsibilities

• Provide safe environment, accommodation for live-in, meals, rest area
• Allow nurse to provide care as booked
• Pay via Paystack, Booking ID is proof

Nurse Consent

I consent to provide professional care as per service booked, keep confidentiality, arrive on time, no extra charges, use SOS if emergency.

Data Consent

I consent to collection of name, phone, email, GPS actual name, photo, health info for care, booking history, stored in Firebase (RTDB + Firestore) with auth != null rules, Anonymous auth enabled, as per Privacy Policy.

Deletion Consent

I understand I can delete account at delete-account.html with reason, GPS actual name saved to deletion_logs for 90 days.

Sign by booking - booking implies consent. For paper form, contact CEO.
